Instruction for successful candidates:

• Physical Check up

• Successful candidates in Bangkok must undertake physical check up in Vibhavadee Hospital between April 16 – 17, 2011 as from 7.00 – 12.00 hrs. (Please contact 2nd floor Physical Check up Department /Fast as least 8-12 hours before check up)
• Successful candidates in Songkhla must undertake physical check up in Sikharin Hospital, Hatyai between April 16 – 17, 2011 as from 7.00 – 12.00 hrs. (Please contact 2nd floor Physical Check up Department /Fast as least 8-12 hours before check up)
• Please bring the 2 page-physical check up questionnaires (attached) to the hospital on the check up day. You can download the forms from the left hand side attachments
  • Page 1 (Physical check up questionnaires) – to be completed by interviewee
  • Page 2 (Health Examination Summary) – to be completed by the Hospital
• Upon completion of the physical check up, the Hospital will submit the 2 pages to the Center for further action.
• Notification will be made within 2 weeks after physical check up.

• Additional instruction and other document eg. Lettter of guarantee Information for the preparation for offshore petroleum skill training and related documentation will be communicated thru each E-mail Address as notified to the Center.
